OMFS accepts most dental and medical insurance. However, if you have
questions regarding the acceptance of your coverage please contact
our office. We
will gladly file your insurance as a courtesy, and accept assignment
of your benefits provided we are able to verify your coverage. However,
if your insurance does not pay after 30 days from the submittal date,
it will be your responsibility to pay for the services and resubmit
the insurance. We will be glad to assist you with re-submittal.
Quoted
benefits are an estimate only, based on the information obtained from
your insurance company. In no way is it a guarantee of payment from
the insurance company. Please
notify our office as soon as possible when you have any changes in
address, phone number, insurance coverage and medical history.

It
is the goal of Sandalwood OMFS to provide you with optimal surgical
care at reasonable cost to our patients. In order to maintain affordability,
total payment is due for services when treatment is rendered.
In the case of insurance coverage, your entire estimated share is
due at the time of service. Any remaining balance on your account
after insurance has paid will be your responsibility.
For your convenience we will accept as a form of payment: Visa/Master
Card, check, cash and Care Credit. Care Credit is a credit card for
health and dental services. For questions regarding Care Credit please
contact our office. We will be happy to assist you with an application.
It is the responsibility of the parent or guardian who is seeking
treatment for a child to ensure that payment is rendered at the time
of treatment. The
practice reserves the right to add a service charge to all accounts
that are sent to collections. |
